[PATCH 14/15] inbox: shrink data structures for publicinbox.*.hide

We no longer vivify the intermediate $ibx->{-hide} hashref,
instead we use $ibx->{-hide_$KEY} directly.  This avoids
an intermediate hashref and extra hash table lookups.
