Would it make sense to mirror the previously existing condition and 
check for is_new instead?  I.e.:

			if ((!patch->is_delete && !patch->new_name) ||
			    (!patch->is_new    && !patch->old_name)) {

or

			if (!(patch->is_delete || patch->new_name) ||
			    !(patch->is_new    || patch->old_name)) {
